Unexplained charges, disconnected service, auto pay doesnt work

So I have Verizon FiOS TV and Internet, when I signed up I was offered a free HD box for 12 months and HBO package for 6 months as part of a promotion they were running.
The first month itself I was charged for the HD Box plus a DVR ( that I never had/requested) and a South Asian Channel Package.

I called and after about 2 hours of being on the phone with them they reversed those charges.

Three months later those same charges started appearing on my bill again. So I called back, another two hours and the charges were reversed.

The month after that, when the same thing happened again I requested they disconnect my service in 10 days ( from the day I called, as against immediately).
I put the phone down and get online to check my email and my connection was gone.

I called back, after 4 transfers and about 45 minutes a supervisor tell me that to get my services restored I must either apply for a new connection ( which requires a credit check) or pay 250 as deposit because of my bad payment history.

When I pointed out that I had no late payments and was enrolled for auto payments, she said "wait a second let me take a look..oh its because you have too many adjustments"..too many adjustments??? that is because you have charging me the wrong amounts conistently every month since I signed up with you.

She said she will explore options and call me back in an hour, I didnt hear back all day. So I called back and guess what the supervisor I spoke to yesterday was off and the new lady said she couldnt do anything better than have me re apply or pay the deposit.

I have never ever ever ever seen customer service worse than this. I dont ever want to deal with verizon again, and I wish that you dont either.
